You typically do not need a referral to see a Primary Care Provider (PCP) but it is best to check with your insurance company prior to your appointment.

We accept most major insurance plans. Please call our office to confirm we bill your plan.

It is the policy of Family Practice Associates to provide essential services regardless of the patient’s ability to pay. Discounts are offered on a sliding fee schedule based upon family income and size.

Please request an application, complete all the required information, and return the completed forms to the office. Family Practice Associates will then process your documentation and review the information provided to determine if you, or members of your family, are eligible for a discount.

We require that if a patient is more than 15 minutes late for an appointment they will need to reschedule, and not for a same day appointment. 

We require a 24-hour notice for an appointment cancellation. If you cancel your appointment in less than 24 hours, it is considered a no-show.  After 3 no-shows you may be discharged from the practice.

Your physician will communicate your test results with via our patient portal.  If you do not hear from our office within 5 days after having a study or lab work completed please call our office. 

The best way to have protected time to discuss your health concerns with your clinician is to schedule a visit.  You can schedule an in office visit, telemedicine visit (when appropriate) or telephone visit (again when appropriate).  You can also call the office and speak with one of the staff to have them relay a message to your clinician.  Please note clinicians are not able to take phone calls during the day as they are seeing patients in the office.  We always try our best to get back in touch with patients as soon as possible.

If you are experiencing a medical emergency or what you believe is a life-threatening situation, call 911 immediately or go to the Emergency Department of your nearest hospital.

Yes, you can call ahead, we will pull them for you and you can pick them up.

Yes, we set aside appointments each day for urgent visits. Please call at 7:35 AM to schedule a same day appointment if you need to be seen.
